Declaration for Original Invalid Pension

State of Maryland, City of Baltimore

On this 11th day of December 1889, person appeared...  Albert A. Allard, aged 64, a resident of 1411 E. Biddle Street, city of Baltimore, state of Maryland, who being duly sworn according to law, declares that he is the identical Albert A. Allard who was enrolled in August 1861 in Company I of the 4th Regiment of Vermont Infantry commanded by L.A. Stevens and was honorably discharged at Camp Griffin, Va. on the 8th day of January 1862; that his personal description is as follows:  Age 64 years; height 5 feet 8 1/2 inches; complexion fair; hair dark; eyes dark.  That while a member of the organization aforesaid, in the service and in the line of his duty at New Bern, in the State of North Carolina on or about the 2nd day of February 1864, he bring on board of the USS Underwriter, during a night attack of the enemy the ship was destroyed and I with the most of the crew were captured; from exposure to the weather caught a severe cold; not having sufficient clothing; and from severe strain from coughing ruptured myself on the right side.  That he was treated in hospitals as follows: on board USS Commodore Hull.

That he has been employed in the naval service otherwise as stated above subsequent to the above service shipped on the Alleghany, USN July 1862 transferred to the Underwriter.  Whilst on board, the ship was destroyed by the enemy.  Was thence transferred to the USS Commodore Hull.  Discharged June 6, 1865 at Brooklyn, NY.  That since leaving the service, this applicant has resided in the City of Baltimore in the State of Maryland, and his occupation has been that of a Tinner.  That prior to his entry into the service above named he was a man of good, sound, physical health, being when enrolled a Tinner.  That his is now partially disabled from obtaining his subsistence by manual labor by reason of his injuries, above described, received in the service of the United States; and he therefore makes this declaration for the purpose of being place on the invalid pension roll of the United States.  He hereby appoints the full power of substitution and revocation George W.F. Vernon, 108 St. Paul Street, Baltimore, Md, his true lawful attorney to prosecute his claim.  That he has not received or applied for a Pension.  That his Post Office Address is 1411 E Biddle St, city of Balitmore, State of Maryland.  Signed Albert A. Declaration for Invalid Pension

State of Maryland, City of Baltimore

On this 12th day of January, 1892, personally appeared... Albert A. Allard, aged 55 year, a resident of the city of Baltimore, State of Maryland, who, being duly sworn according to law declares that his is the identical Albert A. Allard who was enrolled on the 15th day of August, 1861 in Co. I in Vt. Infantry as 2nd Lieutenant; resigned in February 1862; reenlisted May 1862 in U.S.N. on receiving ship "Alleghany" as landsman; transferred to U.S.S. "Underwriter" & promoted First-Class fireman; transferred to U.S.S. "Commodore Hull" in the service of the United States in the War of the Rebellion, and served at least ninety days, and was honorably discharged at Brooklyn Navy Yard, New York, on the 6th day of June, 1865.  That he is now totally unable to earn support by manual labor by reason of Hernia, asthma, disease of heart, varicose veins, rheumatism, and general nervous debility. 

That said disabilities are not due to his vicious habits and are to the best of his knowledge and belief permanent; that he is a pensioner under Certificate 11709.  That he makes this declaration for the purpose of being placed on the pension-roll of the United States under the provisions of the Act of June 27, 1890.  He hereby appoints, with full power of substitution and revocation, Frank Nolen, 102 Lexington St., Baltimore, State of Maryland, his true and lawful attorney to prosecute his claim, and to receive therefore a fee of ten dollars; that his post-office address is 1411 E. Biddle St., city of Baltimore, Status of Maryland.

In answer to yours of 21st instant, in the case of Albert A. Allard, Lds., you are informed that he enlisted July 14/62, served on "Alleghany" to July 25/62; on "Underwriter" from July 26/62 to Feb 1/64; on "Comm. Hull" from Feb 2/64 to June 2/65; when discharged.


Navy Department, Bureau of Medicine and Surgery - Washington, 14th July, 1890

In reply to your communication of 13th July 1890, relative to the disability of Albert A. Allard, Lds., U.S. Navy, you are informed that the records of this Bureau show as follows, viz.:

"Commodore Hull" admitted 28 March 1864 with abscesses of frontal sinus.  Purgatives.  31.  Has violent pain over eyebrow on left-side.  Large discharge of mucus from the nostril on that side.  Feels better as the discharge increases.  Have a number of cases of the same kind.  Origin not stated.  Discharged 2 April 1864, to duty.  Journals of the "Underwriter" are not on our file: and those of the "Alleghany" do not afford any information in his case.

Answer.  On the first April day 1860 by the Rev. Geo. F. Adams, Pastor of the 2nd Baptist Church at Baltimore.

Answer.  I know of none except the Bible

Increase Navy Pension Claim No. 11709

We hereby certify that in compliance with the requirements of the law we have carefully examined this applicant, who states that he is suffering from the following disability, incurred in the service viz:  Hernia, Asthma and disease of the heart, Varicose veins, Rheumatism and general nervous disability.  And that he receives a pension of 8 dollars per month.

He makes the following statement upon which he bases his claim for Increase.  Hernia and Asthma contracted in the Service - disease of heart had for about 20 years - varicose viens since 1865 - Rheumatism for about 15 years; nervous disability for about 5 years.

Upon examination we find the following objective conditions:  Pulse rate 66; respiration 18; temperature 98; height 5 feet 9 inches; weight 154 pounds; age 56 years.
Hernia: - right oblique inguinal Hernia size of tumor 1 1/2  inches in diameter - it passes through the external abdominal ring - ring palidous 3/4 inch in diameters - can be reduced and retained by a properly fitting truss.  - ten eighteenths

Asthma and disease of heart: - respiration slow and labored on taking exercise - apex of heart 1 inch below pericardial line - impulse strong diastolic sound loudest at apex of heart - and sounds of a blowing nature - Asthma we believe to be due to valvular disease of the heart.  We find mitral obstruction.  six eighteenths

Varicose veins: - of right leg - a small cluster on inner aspect middle third of leg about 1 1/2 inch in diameter - not sufficient disease to rate.  - no disability

Rheumatism: - all joints, muscles and tendons at present in normal condition - no disability.

Nervous debility - We can find no indications of said disease existing - what he calls general nervous debility are symptoms produced by condition of the heart - no disability.

He is, in our opinion, entitled to a 10/18 rating for disability caused by Hernia, and 6/18 for that caused by Asthma and Heart.

We hereby certify that in compliance with the requirements of the law we have carefully examined this applicant, who states that he is suffering from the following disability, incurred in the service viz:  Rupture of right side - result of coughing.  And that he receives a pension of 0 dollars per month.

He makes the following statement upon which he bases his claim for Original.  Claims to have pain and dragging in rupture - preventing hard labor.

Upon examination we find the following objective conditions:  Pulse rate 62; respiration 17; temperature N; height 5 feet 6 inches; weight 150 pounds; age 63 years.
General physical condition is below par.

Right inguinal hernia - indirect incomplete marked impulse on coughing.  Intestine presents at the external ring but does not protrude - receding as soon as cough ceases.  Tumor is 2 inches long and one in diameter.  Easily reducible.  Ring slightly patulous.  Can be retained by truss. 

Heart and lungs are normal

no other disability.  No evidence of rupture on left side.


General Affidavit - State of New York, County of New York

In the matter of original invalid pension claim no. 15524 Navy of A.A. Allard, Late Co. I 4th Infantry Vt., and U.S.S. Underwriter U.S.N.  Personally came before me, a notary public in and for said County and State of New York, Joseph Saunders, aged 50 years citizen of New York, well known to me to be reputable and entitled to credit, and who, being duly sworn, declare in relation to aforesaid case, as follows:

In the early morning while yet at quarters - on or about February 2nd or 3rd 1863 or 4, A.A. Allard with a few of the Underwriter Crew came on board of the Commodore Hull - As we were short handed - They were mustered with our ship company.  Allard became sick with a cold.  His head swelled.  Body sore.  Could hardly bear touching.  Coughed and strained.  Complained that he was ruptured. - He was a long time on the sick list.

Joseph Saunders - 493 6th Ave, NY City, late 1st Class Fireman, US Steamer Commodore Hull

U.S.S. Commodore Hull.  On or about the 2nd of February 1863 while lying at the mouth of Trent River, Pamplico Sound, NC - Attack was made on U.S.S. Underwriter - lying opposite Fort Totten about 2 O'Clock in the morning - in which the ship was destroyed.  Her crew were either killed or captured.  Some made their escape - A.A. Allard among the latter. - He was transferred to our boat, the Hull - was shortly after taken sick - his head swelled so that one of his eyes closed, a heavy cough set in, he complained very much with.  Pains in his limbs.  I understood from him he was ruptured.  He went ashore in New Bern.  Got a truss & put it on.

General Affidavit - State of Maryland, City of Baltimore

In the matter of original invalid pension claim no. 15524 Navy of A.A. Allard, Late Co. I 4th Infantry Vt., and U.S.S. Underwriter U.S.N.  Personally came before me, a Justice of the Peace in and for said City and State of New York, Robert Davis, aged 55 years citizen of Baltimore, well known to me to be reputable and entitled to credit, and who, being duly sworn, declare in relation to aforesaid case, as follows:

I certify that I know the above claimant, A.A. Allard - he was with me on the U.S. Gunboat Underwriter as Fireman and was taken prisoner at the time of her capture by the enemy February 2nd, 1864 and escaped from her the same night by swimming and was placed on board the U.S. Gunboat Commodore Hull again in company with me and went under the Doctor care complaining of a very bad cough with caused a rupture of which I was an eye witness.

I was a shipmate of the claimant.  On board of the U.S.S. Underwriter and the U.S.S. Commodore Hull.  We were discharged together at Brooklyn, NY.  We then went to New York City, and stopped at the same hotel this in June 1865.  About the 6th instant I do know of my own personal knowledge and intimate acquaintance with the claimant that prior and at his discharge he suffered from rupture right side.  I know the above facts from being with him in the same ships of the U.S.N. and sleeping with him in New York after our discharge in June 1865, leaving New York, shortly afterwards for Baltimore.  Seeing the claimant at least once a week from 1865 to date 1890, and seeing the rupture which is on the right side from my intimate association with the claimant I have seen the rupture fifty times or more.

Sir:  In reply to your questions in regards to my knowing of A.A. Allard's disability is that we were shipmates on the U.S. Gunboat Underwriter and was in good health up to the time of her capture by the enemy.  He was tied and placed on deck with others in the rain and afterwards made his escape and was afterwards put on the Gunboat Commodore Hull as well as myself.  He had a very bad cough and from the cough a rupture was caused.  How I know the rupture was by his showing it to me at the time.  He and myself were paid off and discharged at the same time at the Brooklyn Navy Yard in 1865.
