Troop Forms every Troop needs

Girl and Adult Health History

You may ask each girl and adult volunteer to complete this form.  It is required for participation in physically demanding activities (water sports, horseback riding, skiing, etc.).  Any trip lasting more than three nights requires, in addition to a health history, a health examination (must have been within the last two years).

Girl Scouting is a Family Affair

This form replaces the Parent Interest Form.  Although it is not required, it encourages parent participation.  Use this form and catalog your parent interests, talents and willingness to help with various activities.

Outstanding Troop Award

Don't wait until April to begin to fill out this form!  Every troop should try to become an Outstanding Troop.  Start early to decide how you are going to achieve each of the requirements.  If you have questions about how best to work towards these goals, contact our Awards and Training Coordinator.  This form must be submitted to the District Managers before April 30, 2008.

Annual Troop Report 

You must submit this report with your Outstanding Troop Award form by April 30, 2008.  If you are not submitting an Outstanding Troop Report, you must bring the completed report to the May 12, 2008 meeting.  You need to include your most recent bank statement with this report.  For information about troop bank account procedures, click here.

Detailed Cash Record

This is a tool that you can use all year long to help complete the Annual Troop Report.  This is not a required form, but troops may find it helpful keeping track of their finances.

Cookie T1-A Worksheet

This worksheet can be used for calculations only.  Parent signatures should still be obtained on the original T1-A form provided with troop cookie packets.

Cookie T2 and Incentives Reports

These worksheets can be filled out and returned for your initial and final reports and all girl incentives.  There are four excel spreadsheets within this report (note tab names at the bottom).

Community Service Project Record

Every time your troop participates in a community service project, you need to complete this form.  Bring the form to the next district meeting and return to the membership specialist.  Online registration is also available

 

Troop Trip Forms

Trip Progression Chart

This is a great resource for leaders to ensure that they have proper training, paperwork and insurance for various types of trips.

Parent Permission Form

Any time you are taking your trip away from your normal meeting place or if you have an overnight event (including at your normal meeting place) you should have parents complete this form.  You can save this Microsoft Word document and edit it for each trip.

Troop Trip Application

You need to complete this form for any overnight trips except the following:  District events, GSHNC Service Center events, overnights at your sponsorship, and overnights at a leader's home.  For clarification on a specific overnight activity contact your district managers.  This form should be completed and returned to council at least one month prior to the event.

Program Event Registration

Use this form to register for a GSHNC event listed in FYI.  All fees and the completed form must be returned to council by the deadline date in FYI.

Individual Program Event Registration

Use this form for an Older Girl that is the only participant for a troop for an event sponsored by GSHNC.

Juliette Program Event Registration

Use this form for an Juliette that is individually registered and wished to be a participant for an event sponsored by GSHNC.

Campsite Registration

This form must be completed and returned to council as early as the following dates:  September 15, 2007 for any rentals from April 2008 - August 2008 and May 15, 2008 for any rentals from September 2008 - March 2009.  The facility deposit must be included with the reservation and the usage fee must be paid in full no later than 4 weeks prior to the event.  Cancellations must be made in writing at least 4 weeks prior for a refund less a 15% processing charge.

Site Usage Fee Schedule

This schedule lists all available council campsites and rental facilities and the cost for each for a 24 hour period.  It shows the type of lodging and the capacity of each facility.  Some facilities are available to rent 1/2, 1/3 or 1/4 of the unit.  Deposit amounts for each facility are listed.

Troop Facility Usage Report

This form should be completed after each trip for which you submitted a Troop Trip Application.  You need to complete parts A and B for all trips and part C if you used a GSHNC site.

Equipment Registration/Facilitated Program Form

To reserve council equipment complete the form and return to GSHNC Equipment Reservationist.  You must return the form with a deposit 4 weeks prior to the date you need the equipment.  Equipment will not be reserved without a deposit and you will be contacted if the equipment is not available.  You need to write separate checks for the equipment fees and the deposit.

Resource Center - Equipment Reservation Form

This form is used to reserve items for ceremonies (including flags, stands, candles, etc.), camping equipment, cookie costumes, resource books and other miscellaneous items.  Most items rent for free, but require a $20.00 deposit.

Borrowed Vehicle Contract

This form should be filled out whenever you are using a borrowed van or bus (even if there is no cost).  The form should be completed and submitted with the troop trip application.  This does not apply to leaders and parents personal vehicles.

Girls are IT Program and Bus Registration Form

Use this form to reserve the Girls are IT bus at council or to use the bus at another site.

 

Leader Forms

Training Registration

Use this form to register for any training courses.  The form must be filled out and returned 7 days before a class.  ***Note:  You can register for most classes online.

Adult History Form

This form lists all training you have received, positions (both troop and district) you have held, # of years of service as a girl and adult and any awards you have received.  You may be asked by the Awards and Training Coordinator to complete or update this form at district meetings.  Click here to see available adult awards and information about qualificiations.

I'm Trained Order Form

This is an optional patch series that you may order to show the trainings you have completed.

Membership Numeral Guard and Years of Service Pin

The Membership Numeral Guard is for years as a girl and an adult.  The Years of Service Pin is only for service as an adult.  If you have served or completed years as a Girl Scout (at least 5, then in 5 year increments), complete this form and return to our Awards and Training Coordinator (or bring to a district meeting) by March 1, 2008.

Outstanding Leader Nomination Form

Do you know an exceptional leader that you would like to nominate as an outstanding leader in our district?  Complete this form and return to our Awards and Training Coordinator at (or bring to a district meeting) by March 1, 2008.  You need to have two letters of reference for the leader.

Outstanding Volunteer Nomination Form

This award is for any volunteer has made an extraordinary contribution on either a troop or district level in a position other than troop leader.  Complete this form and return to our Awards and Training Coordinator at (or bring to a district meeting) by March 1, 2008.

Leadership Development Pin

This award is for any leader, assistant leader or group coordinator that has completed Getting Started, Orientation, Age-Level Training, First Aid/CPR, Exploring Outdoors, Troop Preplan and Overnight.  The candidate must have been in their position for at least one year and has participated in at least two meetings beyond troop level (district meeting, district event, annual meeting, open membership meeting, etc.).  Enrichment training leaves may be earned with additional training.  Complete this form and return to our Awards and Training Coordinator at (or bring to a district meeting) by March 1, 2008.

Board Level Letter of Endorsement Cover Sheet

Use this cover sheet for endorsement letters for all board level awards:  Appreciation Pin, Honor Pin, Thanks Badge and Thanks Badge II.  The packet needs to be to council by January 15, 2008.  Contact our Awards and Training Coordinator with any questions.

District Event Forms

District Event Proposal

This form should be completed by anyone coordinating a district event 2-3 months prior to the event and returned to the Membership Specialist.

District Event Report

This form should be completed by the event coordinator and returned to the Membership Specialist within 2 weeks after the event.  The coordinator of the event should try to complete the form at the event because much of the information is difficult to compile after it's over.

 

Troop Forms your Troop may need

Girl Assistance Request Form

This form is for assistance for girls who would not be able to attend council, district or troop events because of financial hardships.  This should be completed and submitted at least one month in advance of the activity.  If the form is not completed in it's entirety, it may be returned.  If the event is a council sponsored, you must submit this form with the registration form.  Use an additional form if you are requesting assistance for more than five girls.  The information submitted will be kept confidential.  Registration can also be completed online.

Registration Assistance Form

Use this form if any girls you are registering for annual membership are requesting financial assistance.  You must submit this with your dues summary and registration forms.

Incident/Accident Report

This should be completed after any accident, medical emergency or any action that was taken (including correction behavioral issues) that could affect the safety or health of the girls.  If you are unsure if you need to complete this form, contact district managers for guidelines.

Consent forms for Girls and Adults

This form gives Girl Scouts USA permission to use and duplicate videos, artwork, photography, audio or writing however they see fit.  It clears the originator of any claim to the material if it is used.

Sensitive Issues Parent Permission Form

This form should be used as a parent permission slip for any program that contains information that may be considered a sensitive issue.  The form stresses that program participation is optional.

Sensitive Issues Program Planning Form

This form needs to be completed and returned to council at least 30 days prior to any program that contains information that may be considered a sensitive issue.  A Sensitive Issues Parent Permission form should be obtained for all girls that participate.

Troop Earning Application

This form should be completed and returned one month prior to any troop "money earning" activity.  This does not include Fall Sales or Cookie Sales.

Troop Disbanding Report

If your troop is disbanding, you must complete this form and send to GSHNC.  You must also include any money, checkbook and banking information.
